To participate in the program and request a Hats Off To Veterans starter kit please send an email to

with “Hats Off” in the subject line, or write us letter and mail it to:

Reminder: Don’t forget to include your postal mailing address with your request so we know where to send your kit!
Before registering for the Hats Off to Veterans individual program we ask those who can afford it to consider making a voluntary contribution of $11 per registration to help us defray the costs of the program. Donations are completely optional. All gifts receive acknowledgment and a tax receipt.

Be sure to include a your name, address, phone number and a message stating that you want your gift to go to My Soldier/Hats Off.
Please note there is no form for you to complete. Based upon the amount donated we mail out the appropriate contents with your starter kit. When we see a donation in the amount of $11.00 we automatically process it as a Hats Off Mini Kit. The kit contents will include a Veterans contact and address, guidelines, greeting card, and a My Soldier hat for your Veteran.
